For years, life sciences organizations have relied on retrospective data to understand patient populations. Claims, registries, and historical electronic health record (EHR) data delivered valuable insights, but often after the window for intervention had passed.

As therapies become more personalized and eligibility criteria more complex, the challenge is no longer understanding who could benefit. It’s identifying the right patients while clinical decisions are still being made.

That’s the focus of a new partnership between Modular Medical and IMO Health, aimed at identifying patients with diabetes who may benefit from a new tubeless insulin pump – directly within the EHR.

Clinical guidelines often recommend insulin pump therapy for patients who require intensive insulin management – particularly those with persistently elevated A1C levels, frequent hypoglycemia, or difficulty achieving glycemic control on multiple daily injections (MDI). But determining who may benefit often requires clinicians to piece together information scattered throughout the medical record, including treatment history, documented challenges, glycemic trends, and patient preferences.

Rather than relying on manual chart review or broad diagnosis codes alone, IMO Health’s clinically curated terminology translates these nuanced clinical characteristics into deployable phenotypes that can identify appropriate patients in near real time during routine clinical workflow.

Teams increasingly need ways to move beyond retrospective patient finding toward workflow-integrated identification. Whether supporting clinical trial recruitment, accelerating therapy adoption, or improving access to innovative treatments, success depends on recognizing clinically relevant patients when providers are making care decisions, not months later.

By combining expert clinical terminology with deployable algorithms, organizations can reduce manual effort, improve consistency across health systems, and help connect more patients with appropriate therapies.
